Register with us as a New Patient

How to Register

You may register as a patient with us if you live within the practice area (G41, G51, G52 and G53 postcodes) and are entitled to NHS services.

In person

All patients will be asked to complete a registration form with details of previous GP and home address. We also ask you to fill in a brief health questionnaire asking for information about previous illnesses, medication, smoking, weight and ethnicity.

Each family member must complete a separate form and the person with parental responsibility needs to complete and sign forms for children younger than 16 years. All completed forms should be returned to reception.

Please bring in your passport or other form of photographic ID, national security card and proof of address (e.g. a recent utility bill).

Completed applications take 24 hours to process. Access to practice services can be made after this time. Your previous GP records will normally take up to 8 weeks to be transferred to us.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

It is is important to note that you can only permanently register with a GP practice if you’re staying in the area for more than 3 months.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Register online

Step 1: Complete online registration form

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Step 2: Upload scans of photo identity and proof of address

Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

Once you have completed the form please upload the form to the practice.
